To Quran teachers: what do you do when a mistake keeps coming back?

Salam alaikum, I have a question for Quran teachers who run hifz circles. How do you handle a mistake that a student keeps repeating even after you’ve corrected it more than once? Does your approach change depending on whether it’s a pronunciation issue, confusing similar ayahs, or mixing up the order? And if you’ve faced this with several students, have you come up with a way to prevent it from the start of memorization?
