Update: My Battle with Cancer (Alhamdulillah for Strength)
As-salamu alaykum. I was diagnosed with PNET on June 7th, 2025 at age 42. I live in Bangkok with my wife and our 2-year-old son. It’s been a huge emotional ride for all of us - we first thought it was PDAC and were told I might have less than a year, then we learned it’s a neuroendocrine tumor and were told I might have 3–5 years. Alhamdulillah for every day. - UPDATED Nov 11 Since June I’ve had 2 rounds of chemo and 3 rounds of PRRT with Lutetium, because my cancer cells have the right receptors. I’ve also had 2 rounds of RFA on the pancreas to remove primary tumors; those were largely successful. Everything happened quickly once we started treatment. Liver labs have been a bit up and down: GGT: 813 → 603 → 478 → 999 → 1,674 → 1,263 (some decreases that could mean improving blood flow) ALP: ? → ? → 126 → 176 → 259 → 257 ALT: 322 → 170 → 37 → 41 → 83 → 53 (back toward normal) AST: 53 → 68 → 67 → 69 → 107 → 95 (signs of reduced stress on liver) Tumor markers: CA 19-9: 2,384 → 743.8 → 629.3 → 738 → 1,212 (still below the very high baseline; RFA may affect values) CEA: 11.1 → 7.4 → 6.1 → 6.7 → 6.7 (fairly flat, which supports stability) Updates: I’ve had another RFA session. My specialist mentioned that repeated ablations sometimes trigger an immune response that could help fight the cancer, and RFA can also help restore some pancreatic function, so we repeated it. I also had a scary episode at breakfast where I had seizure-like symptoms and basically blacked out for about 30 minutes. My wife and I ended up in the ER and I spent a day under observation in ICU. I lost responsiveness and some control of my body - it was terrifying for both of us. What’s next: I’ll have a PET MRI to look for any lesions in the liver that might be treatable locally later. Better imaging will help us make informed decisions on tumor progression and next steps. For now we’re on a palliative-focused plan until the liver improves enough or until more treatment options become available. Next round of PRRT plus SSA treatment is planned for January, inshaAllah.
